We only surface a finding if there is a specific, reasonable question a customer could ask their provider about it. If we cannot articulate that question, the finding does not belong in your report.
Each finding carries a confidence rating that reflects how clearly the pattern appears on the statement itself, not how likely a provider is to agree.
A high-confidence finding is still a question, not a conclusion. It means the pattern is unambiguous on paper, not that the charge is wrong.
When you tell us you only remember one imaging service, we record that as your recollection and ask the provider to verify it against their records. We never present what you remember as established fact in a document sent on your behalf, because doing so would misrepresent the basis of the dispute.
When a provider response is ambiguous or raises something our process cannot resolve on its own, the case is routed to an internal review queue rather than answered automatically. We would rather pause and look at it than send something inaccurate in your name.
