The same service billed twice on the same visit. This happens most often with lab work, imaging, and medications. Look for the same CPT code appearing on the same date more than once without a clear reason (like a legitimate repeat procedure).
Billing for a higher-complexity or more expensive service than was actually documented. The most common version is an ER visit coded at level 5 (the highest) when the documentation supports level 3 or 4. The difference in billed charges can be thousands of dollars.
One procedure separated into multiple line items that are normally grouped together and billed as a single code. Billing rules require certain services to be bundled, unbundling them artificially inflates the total.
Line items for care you don't remember receiving, or from providers you don't remember seeing. This can reflect genuine errors, documentation mistakes, or occasionally charges from physicians who were present in the OR but had no direct contact with you.
Even if a charge is correctly coded, the price can be far above what other providers in the same region charge for the same service. This is harder to dispute, but worth noting, especially if you're uninsured or the service wasn't covered.
Your coverage exists, but the claim was filed incorrectly, applied to the wrong plan year, or processed without verifying your in-network status. The result is a bill at full price when you should have paid your in-network rate. Always compare your bill against your Explanation of Benefits from the insurer.
