Why You Should Never Pay a Medical Bill on the First Notice

When a medical bill arrives, the impulse is to pay it quickly and move on. After all, hospitals do not hesitate to send accounts to collections. But paying immediately without review is one of the most financially costly mistakes a patient can make.

The Statistics Are Alarming

Multiple studies have found billing error rates in medical bills ranging from 49% to over 80%, depending on the type of bill and the institution. Errors are not rare exceptions. They are the norm. Paying on first notice means accepting those errors as legitimate charges.

The Timeline You Should Follow Instead

  • Day 1: Receive the bill. Hold off on paying. Call and request a 30-day review hold.
  • Day 3–7: Request your itemized bill and UB-04 form.
  • Day 7–14: Request your Explanation of Benefits (EOB) from your insurer.
  • Day 14–21: Compare the itemized bill against your EOB line by line.
  • Day 21–28: Dispute any errors in writing, certified mail.
  • Day 28–30: Confirm dispute has been received and a hold is in place.

What About Collections?

Most hospitals will not send a bill to collections within the first 90–180 days, especially if you have communicated that you are disputing it in good faith. A documented, written dispute puts the provider on notice and creates a paper trail that matters if the account escalates.

Financial Hardship Programs

Before paying anything, ask about charity care or financial assistance programs. Most nonprofit hospitals are required under their tax-exempt status (IRS Section 501(r)) to maintain a written financial assistance policy. Depending on your income and the provider's criteria, you may qualify for a reduction, even on a bill you have already partially paid.
